PROOF OF RESIDENCY:
Proof that you reside within Bremen City must be provided at enrollment by submitting the documentation listed, below.  The student must reside full time with the biological parent/legal guardian whose residence is in the Bremen City.~

 

  ~ Proofs of Residency - Two proofs of residency are required as listed, below

 

Own – A utility bill (gas, water, or electric only) received in your name at the property address, proof of residency from the County Registrar, current vehicle registration showing property address, or a cancelled check in your name for the current month showing property address.

 

Renting – Copy of the lease agreement, a utility bill (gas, water, or electric only) received in your name at the property address, proof of residency from the County Registrar, current vehicle registration showing property address, or a cancelled check in your name for the current month showing property address.

HEALTH FORMS:
ALL STUDENTS entering school in the Bremen School District shall be required to present a Georgia Certificate of Immunization (Form 3231) and Georgia Certificate of Vision, Hearing, Dental, and Nutritional Screening (Form 3300) in compliance with state and local regulations (Policy JGCB).  Parents who do not have proof of immunization will be granted a thirty-day extension to obtain these forms.  Contact a local health department for the hours and times available to obtain form 3132 and/or form 3300:

BIRTH CERTIFICATE:
An official copy of the student’s birth certificate must be presented at the time of enrollment.  

 

SOCIAL SECURITY CARD:
An official copy of the student’s social security number must be incorporated into his/her school records.  If there are objections to the use of the student’s social security number, parent/guardian may elect to waiver.

DISCIPLINE RECORD, OFFICIAL TRANSCRIPTS AND/OR WITHDRAWAL FORMS:
A copy of the student's high school transcript must be provided at the time of enrollment, so that appropriate courses may be selected.  Complete discipline record must also be provided at the time of enrollment.  If the student is enrolling after the semester has begun, a withdrawal form, from the transfer school, is also required.  Placement is tentative until the student’s official transcript has been received to verify grade levels and course credits.  Eligibility for competitive activities is determined by the student’s official transcript, which must be received before participation.

PRIVATE SCHOOL/HOME SCHOOL:
A statement of accreditation must be provided at enrollment, if entering from a private school.  Students entering from a private or home school may have to validate a passing grade by the next sequential course.
